Helping individuals and families address the challenges of serious illness

Life-threatening illness takes its toll not only on a patient, but also on the patient’s family. With funding the Phelps Community Healthcare Foundation, Pathways to Care in the Rivertowns provides compassionate individual and family counseling and supportive connections when people and their loved ones face chronic or life-limiting illness.

Services include:

  • Needs assessment
  • Office and telehealth visits
  • Supportive individual and family counseling from day of diagnosis
  • Discussion and assistance completing advance directives
  • Advocacy (insurance, financial & legal)
  • Assistance navigating complex medical systems
  • Emotional support at end-of-life
  • Resource referrals

Pathways to Care in the Rivertowns is an initiative of WJCS Health & Healing Support Services and open to residents of Westchester Rivertowns, generously funded by the Phelps Community Healthcare Foundation.
